An anal fissure is a tear in the lower rectum's (anal canal) lining that produces pain during bowel movements. Anal fissures do not result in more significant complications.

After a few days or weeks of home treatment, most anal fissures heal. Short-term anal fissures are what they're called. A long-term fissure is defined as an anal fissure that has not healed after 8 to 12 weeks. A persistent fissure can require medical attention.

Anal fissures are a very common occurrence. They afflict people of all ages, especially those who are young and appear to be in good health.

What is the cause of the issue?

An injury or damage to the anal canal causes anal fissures. Injuries can occur when:
  • You walk past a big stool.
  • You're constipated and have a hard stool to pass.
  • You get diarrhoea on a regular basis.
  • You are the one who gives birth. (The anal canal might be damaged during childbirth.)
Fissures are thought to be caused by increased tension in the two muscle rings (sphincters) that control the anus. You have conscious control over the outer anal sphincter. The inner sphincter, on the other hand, is not. This muscle is constantly under strain or pressure. If the pressure is too high, the anus may spasm and blood flow will be reduced, resulting in a fissure. A fissure can also be prevented from healing by this pressure.

What are the signs and symptoms?

An anal fissure might cause the following symptoms:
  • During bowel motions, there is a severe, stinging, or burning discomfort. A fissure can cause excruciating pain. After a bowel movement, it can be brief or persist for several hours.
  • Itching.
  • Bleeding. A small patch of bright red blood on toilet paper or a few drops in the toilet bowl are possible. A fissure's blood is separated from the stool. If you have any bleeding with a bowel movement, tell your doctor.
An anal fissure might be a painless wound that refuses to heal. It may bleed sometimes but has no other symptoms.

What are your options for self-care?

Most short-term anal fissures heal in 4 to 6 weeks with home treatment. Pain during bowel motions normally disappears within a few days of beginning home treatment.

There are various things you may do to alleviate your symptoms and aid in the healing of the fissure:
  • To make bowel movements less painful, take stool softeners or laxatives. Inquire with your doctor about how long you should use laxatives.
  • 2 or 3 times a day, sit in a tub filled with a few inches of warm water for 20 minutes. This is referred to as a sitz bath. It helps relax the internal anal sphincter and soothes damaged tissue. Soaps, salts, and shampoos should not be used in the water.
  • Consult your doctor about using a nonprescription lotion like zinc oxide, Preparation H, Anusol, or 1% hydrocortisone for a limited period of time. These may aid in the relief of anal tissues. Fiber and sitz baths, on the other hand, are more effective in alleviating symptoms. footnote1
  • When it comes to medications, be cautious. Read and follow all of the label's instructions. If your doctor has recommended pain medication, use it exactly as directed. Ask your doctor if you can use an over-the-counter pain reliever if you aren't taking a prescription pain reliever.
  • To clean after a bowel movement, use baby wipes or medicated pads, such as Tucks pads or Preparation H wipes, instead of toilet paper.
Don't put off going to the bathroom. It's understandable if you're worried about how much it'll hurt. However, avoiding bowel motions will worsen constipation and keep the fissure open and painful.

What is the treatment for an anal fissure?

Using stool softeners or fibre supplements, as well as taking regular sitz baths, can help 9 out of 10 short-term fissures heal. In addition, roughly four out of ten long-term anal fissures will heal with home treatment.

However, not all fissures will heal with home therapy alone. If a fissure lasts longer than 8 to 12 weeks, prescription medications may be required. These could include nitroglycerin cream, high blood pressure pills or gels, or botulinum toxin injections (Botox)

If medication does not relieve your symptoms, surgery may be necessary. The lateral internal sphincterotomy procedure is the most prevalent. A doctor performs this operation by cutting a portion of the internal sphincter to relieve the spasm that is producing the fissure.
